At Chelsea Dental Studio, new patients are always welcome. We work to make the process of switching dental practices as easy as possible and approach it with the goal of making you feel like part of the family. Here are a few things you might want to know before you get started.
Your First Visit
The first visit is all about us getting to know you and your needs and you learning about our practice and what we can do for you. We will sit down with you to go over your medical history, dental health, and concerns. No matter what treatment you require, we are confident we can deliver the level of care you need.
Our Treatment Plans
At Chelsea Dental Studio, we don’t believe in shortcuts—and that means no recycling treatment plans. Once we establish your dental health needs, we craft a plan that is fully tailored to you and designed to meet your goals. We ensure you are an active partner in your care, and nothing is set in stone without your final approval.
One way you can speed up your appointment is to arrive with all the paperwork we need filled out and ready to go. Please download and complete all applicable forms prior to your first visit:
New Patient Forms:
New Patient Forms:
- New Patient Registration
- Dental History
- Medical History
- Confidentiality Questionnaire
- Patient Treatment Consent Form

- Regence Blue Shield
- Premera Blue Cross
- Cigna (Radius network)
Time is your most precious commodity, and that is true of all our patients. This is why we focus on seeing everyone right on time. Since late cancellations and missed appointments can throw off our rhythm, we require 48 hours’ notice to cancel or reschedule. A $50.00 fee will be charged to a patient’s account for a no-show or cancellation occurring less than 48 hours in advance.
